Notice of Privacy Practices

My Solace Pointe is required to protect the privacy and security of protected health information, follow the duties and privacy practices described in the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required by law.

We may use or disclose health information for treatment, payment, and healthcare operations, including care coordination, appointment administration, quality improvement, compliance, and practice management. We may also disclose information when required or permitted by law, including public-health activities, preventing a serious threat, health oversight, workers' compensation, judicial proceedings, and law-enforcement requests that meet legal requirements.

Uses not described here generally require your written authorization. You may revoke an authorization in writing except to the extent we have already relied on it. We do not sell protected health information or use it for marketing without authorization where authorization is required.

Telehealth involves electronic transmission of health information. We use administrative, technical, and physical safeguards appropriate to the service. No electronic system can be guaranteed completely risk-free. Do not use routine website forms or email for emergencies.

You may ask to inspect or receive a copy of your designated record set, request a correction, ask for certain restrictions, request confidential communications, receive an accounting of certain disclosures, obtain a paper copy of this notice, and choose someone to act for you when legally authorized. Some requests may be limited or denied as permitted by law, and we will explain applicable rights to review.

We measure public page visits using first-party identifiers and a one-way network hash. We do not capture form contents, clinical information, URL query strings, or raw IP addresses. Browser Do Not Track requests are respected.
